I am tottaly new in Callas and i want to create a Profile plan that covers a CMYK workflow with the following criteria-fixes.
Is there a ready profile plan that covers that or you can advice me how to fix it?

Most of these steps are already included in the predefined profiles for a standard such as PDF/X, which also adds a suffix if the conversion was successful and a valid result was produced.
Furthermore, font embedding is mandatory for PDF/X or PDF/A.
For FOGRA39, you can specify a custom output intent or use ‘ISO Coated v2’.
A process plan should only be necessary if you want to add actions (like image export) or specify the order of these steps. In that case, a simple profile should suffice.

i use the folloing setup , i cant find the rename function although.
My set up is as following any advise?

In the convert to Standard profiles it might not be possible to change and set the fixup.
But you could use with a process plan the “rename PDF” step:
